    How do I file an Appeal, Grievance, or Complaint?
    Please follow these instructions to file an appeal, grievance, or complaint.

     

    How do I register to the web site?
    The following are step by step instructions to walk you through the registration process:

     

    1. On the right hand side of the page, click the Register button.
    2. Enter your personal information which includes: Member ID Number, First Name, Last Name, Date of Birth (mm/dd/yyyy). You also be asked to insert a Code that is displayed in the box below the Date of Birth field for security purposes.
    3. Click the Continue button at the bottom of the screen.
    4. You will then create your Username and Password. Next you will select a Security Question and provide the corresponding Answer – (you will be prompted to answer your security question if you forget your password at a future logon attempt. (Please keep this information in a secure place.)
    5. Insert your email address and select if you would like to receive emails from Anthem regarding your plans and benefits.
    6. If you have Pharmacy Rx with Anthem select the medications you are allergic to and the medical conditions you have. Insert a daytime telephone number we can call with prescription questions.
    7. Choose Continue to begin taking advantage of your online Anthem benefits.
    8. As your last step, for security purposes please confirm the username and password that you set up during registration if you wish to log into the site.
     
    Must I register to obtain information from the web site?
    If you have a policy with us and are a member, we recommend that you register for an account. Upon registration, we will be able to provide you with detailed information around your plans and benefits that is personalized to your coverage.
     
    Where can I find information on the claims that have been processed?
    Click on Check Claim Status and search by your specific claim or view all claims.
     
    Where can I find a claim form?
    Click on Customer Care to access your forms
     
    What if I forget my username or password?
    If you forgot your username, click Forgot Username located above the Username field on the home page. You will be prompted to enter your first and last name, date of birth, cardholder ID and answer your security question before we can provide you with your username. Please remember that your challenge answer is case sensitive.
     
    How can I complete my Health Assessment?
    Click on Health & Wellness to start your Health Assessment.
     
    How can I find a doctor in my network?
    First log into your secure site. Once logged in, click Find a Doctor and answer the required questions to complete your search.
     

    How do I get additional information about my plan or benefits?
    Remember to Log in and access Plans & Benefits to view eligibility, coverage, and deductibles related to your plan. Your Anthem ID card is also a handy resource for copayment amounts and telephone numbers to guide you in the right direction for prescription, precertification and claims filing information.
