Below are materials that provide information on our four new HRA plans that will be available October 1, 2011 – Lumenos HRA 3000D, 3000C, 5000D and 5000C. There are two plan types – Deductible (or standard) and Copay. Deductible plans are indicated by a “D” and Copay plans a “C".
