CU Health Plan - ExclusiveCU Health Plan – Exclusive is a limited network plan that extends throughout Colorado, but is defined by three networks: the Central network (Denver/Boulder metro area), the South network (Colorado Springs area) and the North network (Fort Collins/Greeley area). All members must select a primary care physician (PCP) and your PCP will direct your healthcare as appropriate. PCP changes can be made any time and will take effective the 1st of the month following the date of change.

The Prescription Drug Formulary for the CU Health Plan / Exclusive is limited to Tiers 1 (Generic), 2 (Preferred / Brand Name), 3 (Non-Preferred Brand Name) and 4 (Non-Preferred Specialty Oral and Injectable) only. Certain medications will require prior authorization. Please note that drug formularies are subject to change. If your medication is not listed in the formulary, call Customer Service at 1-800-735-6072 to verify if your medication is covered. You can also request to have prescriptions delivered to your home by using the UCH Mail Order Prescription Service by calling 720?848?1432.
