- As an Anthem Blue Cross member, you are eligible for a HealthyCheck screening once every 12 months.
- If your residence is located greater than 30 miles from a HealthyCheck center, you may be eligible to receive an out of area authorization letter. This letter provides the opportunity to obtain the same HealthyCheck screening services at your primary care physician's office. You will need to call the HealthyCheck department at 1-800-274-WELL (9355) to determine if you qualify to receive the out of area authorization letter.

*Flu shot included if the flu vaccine is in season, per Centers for Disease Control and Prevention (CDC) guidelines and vaccine availability. Flu shots will only be given with HealthyCheck screenings. If the flu vaccine is not in season or available at the time of the member's HealthyCheck appointment, he or she may schedule a return appointment and receive a flu shot at no additional charge if the vaccine becomes available.
For Adults onlya total cholesterol, HDL ("good" cholesterol), and glucose fingerstick screening. For Children only
a hemoglobin fingerstick screening, vision acuity and hearing test, musculoskeletal assessment, and may also include Measles-Mumps-Rubella booster, Polio booster (IPV).

$75 Premium Screening (Available for adults 18 and over)includes services listed in the adult Basic screening, plus

Although most Preferred Provider Organization (PPO) policies provide coverage for annual Well-Woman and Well-Man exams and are not subject to the contract deductible, there are a few Small Group contracts, which fall outside of this provision. HealthyCheck Operations offers to waive the deductible for eligible Small Group members with qualifying contracts if they attend a HealthyCheck screening. HealthyCheck Operations will waive the deductible of both the office visit and lab for members with qualified Small Group contracts* for: Pap smears, mammograms, and PSA (Prostate Specific Antigen) tests.
* Some Small Group contracts provide waiver of deductible for lab work only. Please check your policy regarding coverage.
$25 Fee WaiverAs an incentive for members to participate in a HealthyCheck screening, a $25 fee waiver is offered to select Individual and Small Group members. This offer is only good during the first year of enrollment and is only for one screening per newly enrolled Anthem Blue Cross or Anthem Blue Cross Life & Health Insurance Company PPO and EPO Plan member. A one time $25 credit is applied at the time the appointment is made. Offer good only at HealthyCheck centers, which may not be available in all areas served by Anthem Blue Cross and Anthem Blue Cross Life & Health Insurance Company. Offer is non-transferable.
