Medical Plans
Blue AccessSM PPO 
In- and out-of-network national access with low in-network out-of-pocket expenses. Most provider network choice for your employees. 
Lumenos Consumer-Driven Health Plans  
With these consumer-driven health plans, employees get more control of their health, their health care decisions and their dollars. This can lead to substantial cost savings for you. May be paired with a health incentive account (HIA), enhanced health incentive account (HIA+), health reimbursement account (HRA) or a health savings account (HSA). Lumenos® Deductible-First Health Reimbursement Account Plan (March 2011) 
A new type of consumer-directed health plan in which employees pay for medical expenses out of pocket before accessing the funds in their health reimbursement accounts — saving more of the HRA dollars you contribute. 
Blue AccessSM HSA PPO 
Combines our broadest provider network with the cost savings of a high-deductible health plan. This innovative plan may be paired with a tax-advantaged health savings account. 
Anthem EssentialSM PPO  
Covers the most costly types of emergency room care, inpatient hospital and surgical care, including surgery in a physician's office. A basic plan that offers the most premium savings. 
Blue Traditional®  
Benefits are not based on provider network status. Employees may visit any doctor or hospital. In most situations, they pay a percentage copayment and deductible. 
Blue Preferred® / Blue Preferred® Primary HMO 
Employees must select a primary care physician (PCP), but no referral is required to visit an Blue Preferred in-network specialist. Benefits are generally limited to services from HMO network providers — a great way to save costs. 
Blue Preferred® Plus POS 
Offers employers potential savings while offering employees more flexibility to choose doctors and hospitals.

Dental Blue® PPO (Voluntary and Non-Voluntary Plans) 
Dental Blue offers a choice of two plan networks. Premiums are based on both the network and the plan option selected.
