Lumenos PPO with HRA (Health Reimbursement Account)

Lumenos Deductible First HRA (Health Reimbursement Account)
A new type of consumer-directed health plan in which employees pay for medical expenses out of pocket before accessing the funds in their health reimbursement accounts — saving more of the HRA dollars the employer contributes.
Health Maintenance Organization (HMO)
Plans that cover routine and unexpected care, with fixed out of pocket costs. Employees do not have to select a primary care physician (PCP), and no referrals are required. Benefits are generally limited to services from HMO network providers and co pay based plan options are available. These plans are simple and a great way to save costs.
BlueCare POS (Point of Service)
These plans offer broader coverage than our BlueCare plans with an out-of-network component.
BlueCare Basic POS & HMO
These lower cost plans are available on the POS and HMO platforms. A deductible applies to covered in-network inpatient hospital services and covered in-network outpatient surgical facility charges only. For all other covered in-network services, members pay a fixed copayment.
BlueCare Deductible HMO
This plan is built on our HMO platform and still covers routine and unexpected care, with a fixed out of pocket cost. In addition, this affordable option offers an upfront deductible, followed by copays for most covered medical services.
