Anthem Blue Dental PPO and PPO Plus plans offer the freedom of using an in-network or out-of-network provider with the cost management measures important to employers. These plans are available to both Large Group and Small Group employers with at least ten employees.
Broad, Convenient Provider Network
The Anthem Blue Dental PPO and PPO Plus plans feature access to nearly 70,000 credentialed dental providers nationwide, including general practitioners and specialists. As of 9/1/07, the Anthem Dental PPO network will include 1,389 provider locations. The Anthem Dental PPO Plus network will include 1,564 provider locations. Easy Access
Members may choose a dental provider from anywhere in the U.S. and may visit the dental specialist of their choice without a referral simply by making an appointment.
Although in-network and out-of-network provider reimbursement is the same, by choosing a dentist in the Anthem Dental network, members can maximize their benefits and lower their out-of-pocket costs. In-network providers have agreed to accept our maximum benefit allowance as payment in full, less the member’s coinsurance and deductible. This means that members will not be billed for the difference between a participating provider’s regular charge and the maximum benefit allowance.
More Choices...Less Paperwork
In most cases, diagnostic and preventive services are covered in full. For all other benefits, once you have paid the annual deductible and the appropriate coinsurance, Anthem Blue Dental PPO and PPO Plus plans take care of the remaining charges for covered services provided by network dentists. Plus, network dentists will also complete the paperwork and file claims.
If services exceeding $350 are required, Anthem Blue Dental PPO Plus recommends that your dentist obtain a pre-treatment estimate. That way you know in advance what will be covered by the plan.
