To access, you must login to MyAnthem, select the blue MyProvider tab at the top of the page and click on Find a Provider. Once you have pulled up the provider’s name and information, you will see the Zagat Health Survey.

Anthem Care Comparison to view comparisons of the quality and costs of medical procedures at hospitals and other medical facilities in your area.

To access, login to MyAnthem, click on the 360° Health tab and select Anthem Care Comparison located under the Resources section on the left side.
