An identification card is the key to receiving services covered under your health plan. Your group’s members should carry their identification cards at all times and present them to health care professionals whenever they are seeking treatment. Members may have two different cards — medical/prescription drug and dental. ID cards contain important information for both members and health care professionals.  
Identification cards are to be used only by members of health care plans offered or administered by Anthem Blue Cross and Blue Shield and its affiliated HMOs. Use of an ID card by anyone other than the member named on the card (or covered family members) for the purpose of receiving services constitutes fraud. Penalties for fraud include immediate termination of coverage and criminal prosecution. 
 
Medical/Prescription Drug and Dental ID Cards
Medical/Prescription Drug ID Cards
A medical/prescription drug card contains information that is valuable both to your employees and doctors, like the member’s ID number, group number, benefit information and Member Services phone numbers.  
If your group’s coverage includes an outpatient prescription drug program, this coverage will be indicated on the member’s ID card.  
Typically, your group’s members can present their ID card indicating prescription drug coverage at a network pharmacy, pay any applicable copayment/deductible and do not need to file a claim. However, if the ID card is not presented or the prescription is filled at an out-of-network pharmacy, the member or covered family member will have to pay for the prescription out-of-pocket, and complete a prescription drug reimbursement form for reimbursement consideration.  
Dental ID Cards
If your group’s coverage includes a dental plan, the coverage will be indicated either: 
on a comprehensive ID card that includes medical, prescription drug, and dental information, or
on a separate dental plan ID card

Using ID Cards Outside of Virginia
When your employee is an Anthem HealthKeepers member:
Anthem HealthKeepers provides coverage for care received when your employee or a covered family member is away for an extended period of time, whether it's due to out-of-town business, semesters at school or families living apart. Anthem HealthKeepers members away from home for 90 to 180 days can request a Guest Membership at an affiliated Blue Cross and Blue Shield HMO in their new county/state.  
If a member needs to determine eligibility for a Guest Membership, contact Member Services. A Member Services representative will forward an application to complete. When the application is approved, the member will receive a special membership identification card from the affiliated HMO coordinating care. The member will have to pay any applicable copayments when visiting the affiliated Blue Cross and Blue Shield Network HMO(s) in that area. 
When your employee is an Anthem KeyCare member:
Your group’s Anthem KeyCare members have access to the BlueCard® PPO network*, which expands the in-network doctors, hospitals and other health care professionals from your local area to across the country - even around the world. It is currently made up of over 696,000** participating health care professionals across 240 countries. To receive medical care from a health care professional in this expanded network, members just need to show their ID card. 
When your employee is an Anthem BlueCare member:
Your group’s Anthem BlueCare members have access to the BlueCard network, with over 760,000^ doctors, hospitals and other health care professionals serving Blue Cross and Blue Shield plans. To receive medical care from a health care professional in this expanded network, members just need to show their ID card.
