Information Regarding Office Closures

Anthem Blue Cross and Blue Shield is committed to serving you and our other customers in the Durango and Fort Collins areas. Because of this commitment, we’ll soon implement a new sales and sales support structure. As a result, our offices in Durango and Fort Collins will close, effective May 31, 2005. Until the closings, your contacts in each office are the same.  
Payment Information
If you have a small group or business group of one policy and pay your monthly premium at the Durango or Fort Collins office, please call 800-922-4770 toll free to make your payment. You may pay by electronic funds transfer from your checking account. 
Or, you may mail your payment to us in the envelope provided with your billing statement. Please allow sufficient time for delivery. Otherwise, your payment may not arrive by the due date shown on your statement. 
We’ll post new contact information as soon as it’s available. Thank you for giving us the opportunity to serve you.
