At Anthem we make customer service our highest priority.  
To contact Customer Service, please either: 
Send a question via our secure Customer Service Form.
Call the Member Assistance number on the back of the Member’s ID Card.
 
Customer Service can assist you with a wide range of services, including answering questions about health care benefits, helping find participating providers that are located near home and office, sending claim forms and updating your address.  
Normal business hours are from 8:00 a.m. until 6:00 p.m., Monday through Friday.  
When checking on the status of a claim, please have the following pieces of information ready:  

Member's Identification Number

Patient's first name

Date(s) of service

Name of doctor, hospital, or other provider

Claim number (if you know it)
