In-Network Services 
Blue View Vision offers COVA Care members a wide selection of vision providers including ophthalmologists, optometrists, and opticians. Blue View Vision’s network also includes convenient retail locations, many with evening and weekend hours, including LensCrafters®, Target Optical®, JCPenney® Optical, Sears OpticalSM, and Pearle Vision® Stores. Best of all – each member receives both in and out-of-network benefits. However, members receive the greatest benefits and money-saving discounts by using the network.

Out-of-Network Services 
You can choose to receive care outside of the Blue View Vision network. When visiting an out-of-network provider, you are responsible for payment of services and/or materials at the time of service. After the visit, you will need to complete a Blue View Vision Out-of-Network Claim Form and include your itemized receipt. EyeMed Vision Care is the claims administrator for Anthem Blue View Vision. Once your claim has been processed, you will receive a reimbursement check in the mail from EyeMed Vision Care. We encourage you to view the sample payment materials to help you recognize an out-of-network check in the mail.
