Lumenos is an HSA qualified consumer directed health plan that offers a national network of providers, and is available in Colorado and throughout the United States through the BlueCard Network. Plan members can refer themselves to doctors of their choice, including specialists, both in-network and out-of-network; however, higher out-of-pocket costs are incurred when using out-of-network providers. This plan also covers preventative care at 100% in most cases. This plan does not have a prescription drug formulary under this Prescription Drug plan, but certain medications will require prior authorization.
