BluePreferred PPO offers a national network of providers, and is available in Colorado and throughout the United States through the BlueCard Network. Plan members can refer themselves to doctors of their choice, including specialists, both in-network and out-of-network; however, higher out-of-pocket costs are incurred when using out-of-network providers The Prescription Drug Formulary for BluePreferred PPO is limited to Tiers 1 (Generic), 2 (Brand Formulary) and 3 (Brand Non-Formulary). Certain medications will require prior authorization.
