HMO Colorado is an open access HMO, meaning you do NOT need to get a referral to seek specialty care. HMO Colorado offers in-network coverage only, except in cases of an emergency. The Prescription Drug Formulary for HMO Colorado is limited to Tiers 1 (Generic) and 2 (Brand Name Formulary). Medications listed as Non-Formulary are NOT covered. Certain medications will require prior authorization. *This plan is available to Colorado residents only (see Service Area Eligibility below).
