UA Net HMO

UA Net HMO is a limited network HMO which offers access to the providers and hospitals affiliated with the University of Colorado School of Medicine and the University of Colorado Hospital, including a limited number of community physicians associated with the University of Colorado School of Medicine (“UA Network”). All members must select a UA Net primary care physician (PCP) from the UA Net HMO Provider Directory (Find A Doctor). Specialty care can ONLY be accessed through a PCP referral, and this plan offers in-network coverage only, except in cases of an emergency. All employees/retirees under 65 statewide my elect the UA Net HMO. *This plan is available to Colorado residents only.
The Prescription Drug Formulary for UA Net HMO is limited to Tiers 1 (Generic) and 2 (Brand Name Formulary) only. Medications listed as Non-Formulary are not covered. You can request mail order drugs through Anthem’s Pharmacy Benefit Manager (WellPoint NextRx) at 1-800-962-8192 or the CU Mail Prescription Service at University of Colorado Hospital at 720-848-1432. Certain medications will require prior authorization.
Please note that while referrals are needed for specialists visits, you do not need a referral to see an in-network ob-gyn provider or an in-network ophthalmologist.
Provider search functions using Anthem.com for MyAnthem.com cannot be used to find participating UA Net physicians at this time. To find a participating provider, please click on the “Find a Doctor” link below which will take you to the online directory of participating UA Net Providers.
