The University and its affiliates have seen a prevalence of hypertension and coronary artery disease in their member population. Anthem is offering a number of programs designed to remove barriers to compliance with treatment for these conditions and/or that encourage members to be more compliant with their treatment plans. 
The Condition Centers link below will allow our members to access a tremendous number of self help tools designed to educate and provide a plan of action for whatever condition is applicable to their individual needs.  
The Condition Management Programs link will explain the disease management programs that are available to members to support certain acute conditions. Anthem Nurse Care Managers will work to help our members who have chronic health conditions take charge of their health and improve the quality of their lives. By focusing on prevention, education, lifestyle changes and their doctor’s treatment plan, our disease management programs give our members the tools and information they need to take an active role in their health care 
The disease management programs are voluntary and provided at no additional cost or obligation to you. And, of course, they’re completely confidential.
