It doesn't matter where or why you travel. With the CU Anthem Lumenos plans, you can rest assured that if you need care while out of town, your benefits travel with you.
Should you need to see a doctor while you are out of town, it is to your advantage to see a contracted Blue Cross and Blue Shield PPO provider. These providers are found all over the country. When you obtain covered services from a contracted provided, you can be assured that your services will be paid at the in-network benefit level even though you may be far from home. In addition to having access to in-network services by using a contracted Blue Cross and Blue Shield PPO doctor, you will be protected from balance billing (after any applicable contract limits, coinsurance and deductible) as contracted providers accept Anthem's benefit payment as payment in full. Additionally, these contracted providers will send the claim for their services directly to Anthem, reducing your paperwork and worry about whether the claim was appropriately filed on your behalf.
You are covered for emergent and urgent care at in-network benefits anywhere regardless of the provider's contracting status with Anthem Blue Cross and Blue Shield.
When seeing your provider, please present your Anthem ID card.
To locate contracted providers outside Colorado, please click on the link below and choose “Search the National BlueCard Directory”. When asked for your “identification Prefix”, please enter the three (3) alpha prefix letters listed on your Anthem ID Card, which is part of your full member ID number.

HMO Colorado and UA Net:
Only emergency and urgent care are covered outside the HMO Colorado and UA Net networks.
