Avoid Pended Claims
If you cover a dependent on your plan, you will receive a letter from Anthem after your dependents first claim is filed asking you to verify whether your dependent has other health insurance coverage. This information can also be submitted after 1/1/09 prior to the first claim being paid. You will indicate on the letter whether there is no other coverage or provide the information of the other insurance carrier. If you do not respond, all subsequent claims for your dependent will be pended until a response is received.  
Please click on the link below for access to a COB form to mail your response back to Anthem at 1351 William Howard Taft, Cincinnati, OH, 45206, or you can call your information into customer service at 866-645-4135 from 8:00am to 8:00pm ET, Monday through Friday. We encourage you to be proactive and provide this information to Anthem prior to receiving the request to avoid any delays in claim processing. Effective January 1st 2009, you can set up your own account at Anthem when you register and set up a username and password. After logging into your account, you will be immediately brought to the "home" screen of your account. At the bottom of that page you will see a series of links under the "Other Services" section. Click on "Update your Coordination of Benefits (COB)" link. This will allow you to enter the appropriate information."
