NextRx, our preferred mail service pharmacy, is designed for our members who take maintenance medications, or drugs taken on a regular basis, to treat conditions such as heart disease, diabetes, depression, allergies, and even medications such as oral contraceptives.

Easy Ways To Get Started Today 
Get the most out of your mail pharmacy benefit by taking one of three easy steps: 
Phone. Simply call 888-613-6091, Monday through Friday, 8:30 a.m. to 8:00 p.m. EST.
Mail. Simply download our Mail Service Pharmacy Order Form and mail the completed form along with your original prescription and payment to the address listed on the form.
Fax. Simply complete the Member, Shipping and Payment Information sections of our Physician Fax Order Form. Then ask your physician to complete the Prescription Information section.
 
Your physician can fax the completed form, along with a cover sheet, to NextRx at 800-905-9815. You cannot fax your own prescriptions due to legal regulations. 
Note: Prescriptions for Schedule II Controlled Substances must be submitted via mail.
