Ready to Enroll in a 2010 Plan

We’re glad to hear it. Enrolling is easy because there are no physicals required upfront, and you won’t be denied enrollment for pre-existing medical conditions (except for End-Stage Renal Disease).

How to apply
You can print an application, fill it out and mail it to us. For most plans, you can even enroll online.
First, click on the plan below that you’ve chosen. Enrollment begins on November 15, 2009.
You can enroll in Medicare Supplement plans at any time of the year. Click here to learn more about Medicare Supplement plans from Anthem.
Note: If you are a current member and need to change or dis-enroll from your 2010 Medicare Advantage plan, go to Member Services.

We renew our contract with Medicare annually.
The Medicare Contract is renewed annually, and the availability of coverage beyond the end of the current year is not guaranteed. You are eligible to enroll if you are entitled to Medicare Part A and enrolled in Medicare Part B and you live in the service area. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. With some exceptions you can only enroll during certain times of the year.
To obtain an aggregate number of grievances, appeals and exceptions filed or for full information on benefits, please call the Customer Service Department.
