In response to the growing number of small employers abandoning the health insurance market and demanding more affordable health care coverage for their employees, the State Legislature passed House Bill 03-1164. Effective January 1, 2004, this Bill removed six state-mandated benefits from the Basic PPO Plan: low-dose mammography screening, treatment for non-biologically based mental illness, prostate cancer screening, hospitalization and general anesthesia for dental procedures for children, treatment for alcoholism, and hospice care. The Standard PPO Plan covers these six state-mandated benefits.
