Stateside Coverage with BlueCard®
As an Anthem BCBS plan member, your ID card is your direct link to health services and benefits anywhere in the country. In other words, your health care benefits travel with you!
More than 85 percent of all hospitals and doctors throughout the United States contract with independent Blue Cross and Blue Shield plans. These plans are electronically linked, giving each plan direct access to your benefit information, anywhere in the country.
So the next time you're on vacation or otherwise outside of your local service area and require medical attention, remember to follow these easy steps:
| Call the customer service phone number on your ID card to verify available coverage or check your subscriber certificate for details and requirements.|
Important: Under some health care plans, you must notify your primary care provider of any emergency care within 48 hours of receipt of services. Your plan may also require you to precertify any inpatient services as well as certain outpatient services.

| Visit a participating Blue Cross and Blue Shield BlueCard® provider or hospital. The provider will quickly verify your membership coverage with us by using our special three-digit alpha-prefix which is located on the front of your ID card.|
After you receive care, your claim will be electronically routed to us. We will then process your claim and apply all coverage benefits and savings. You are still responsible for any copays, deductible and coinsurance payments for covered services.
Worldwide Coverage with BlueCard®
Now more than ever, the Blue Cross and Blue Shield symbols are achieving an international presence. BlueCard® Worldwide, an extension of Blue Cross and Blue Shield provider networks, allows you to receive covered inpatient, outpatient and professional services from participating hospitals in countries around the world when you are traveling or living* abroad.
As an Anthem Blue Cross and Blue Shield member, you pay nothing more than the out-of-pocket expenses that you pay domestically for in-hospital services. The hospital will file the claim. You would still be responsible to pay for services which are not covered under your health plan, and for deductibles, copays and coinsurance. For outpatient and doctor care, you will need to pay the provider first. Then complete and send an international claim form to the BlueCard® Worldwide service center.
When you are traveling or living* outside the United States and require hospital or medical care, all you need to do is follow these easy steps:
STEP 1. Call the toll free customer service number on your ID card to verify available coverage, or check your subscriber certificate for details and requirements. (If you are outside the U.S., please call 1-603-695-7000.)

to find out which hospitals or doctors are participating in your international destination.
STEP 3. Go to the nearest participating health care provider in your international location and present your ID card. Be sure to call us in advance to precertify any planned hospital stays.
