Popular retail locations, such as LensCrafters®, Target Optical®, Sears OpticalSM, JCPenney® Optical and Pearle Vision® – offering convenient evening and weekend hours
Once you find a network provider near you, all you need to do is: 
Make an appointment with a network provider and present your ID card at the time of service.
Pay your copayment or any remaining balance (if applicable).
And if you choose to visit a non-network provider, you can file an out-of-network claim with us for reimbursement. 
Outstanding savings program 
In addition to rich benefits for your glasses or contacts, you also get:  
20 percent off the balance when you select a frame greater than your plan allowance
