Blue View VisionSM
Protect your vision. Protect your health.
Regular eye exams are important to health, safety and learning. Even if you think you have 20/20 vision, it’s important to be screened regularly ― at every age.
Eye exams can offer a glimpse into major health conditions, like diabetes, high blood pressure and high cholesterol.1
Nearly one in four school-aged children have vision problems that are not detected by school eye screenings.2

A great choice of providers
Blue View Vision insurance has one of the country’s largest vision networks, with more than 44,000 providers and provider locations, including:
Independent optometrists and ophthalmologists
Popular retail locations, such as LensCrafters®, Target Optical®, Sears OpticalSM, JCPenney® Optical and Pearle Vision® – offering convenient evening and weekend hours
Once you find a network provider
near you, all you need to do is:
Make an appointment with a network provider and present your ID card at the time of service.
Pay your copayment or any remaining balance (if applicable).
And if you choose to visit a non-network provider, you can file an out-of-network claim with us for reimbursement.
Outstanding savings program
In addition to rich benefits for your glasses or contacts, you also get:
20 percent off the balance when you select a frame greater than your plan allowance
15 percent to 40 percent off materials not covered by your plan (i.e. extra pairs of glasses or conventional contacts, lens treatments)
As an Anthem member, you can receive discounts of 15 percent to 20 percent and free shipping on contact lens orders. But, glasses or contacts may not be the answer for everyone. That’s why we offer further savings with discounts on refractive surgery. Pay a discounted amount per eye for vision correction. For more information, visit the SpecialOffers
web site and select Vision, Hearing & Dental.
