Anthem HSA-qualified HDHP
High-deductible health plans are designed to accompany the newest health care innovation—a health savings account. An Anthem HSA-qualified high-deductible health plan (HDHP) paired with a health savings account (HSA) can help protect eligible employees and their families against catastrophic health care expenses.
Network Advantages. Employees will have access to one of the largest networks in Nevada and across the country. Our HDHPs also offer out-of network benefits, allowing employees to make decisions about their doctor, their care and their costs—when and where they need treatment.
Referral-free Specialist Care. With our HSA-qualified HDHPs, employees never need a referral to see a physician specialist for a covered service. However, employees may control their costs and receive the highest level of benefits by choosing specialists from our preferred provider network.
Coverage While Traveling. With our high-deductible health plans, employees have access to more than 600,000 providers and 5,900 hospitals nationwide and around the world through our BlueCard® program. For care away from home, employees should:

| Call their physician for advice about appropriate urgent care treatment.|
| For emergency care, call 911 or go to the nearest health care facility.|
| Contact their physician within 48 hours after receiving services or as soon as reasonably possible to coordinate follow-up care.|
| There are no copayments.|
| Except for certain state-mandated preventive care benefits, employees must meet the deductible before they begin receiving coverage.|
| Once the deductible is met, benefits are based on coinsurance, which ranges from 70 percent to 100 percent for in-network covered services.|
| Based on the plan design, prescription drug coverage may or may not be included. Some Anthem plans feature an insured drug benefit, while others offer a drug discount card.|
| Once the out-of-pocket maximum is met, Anthem pays 100 percent for in-network covered services under all its HDHPs.|

Our HDHPs offer basic health care benefits―preventive care, urgent and emergency care, and coverage for hospital treatment and surgery―plus many "extras" once the deductible is met:
| If the employer offers a prescription drug benefit, employees may obtain prescription drugs from a large network of independent and chain-store pharmacies, plus economical mail-order drugs|
| Or, employers may choose to offer employees a no-cost drug discount card provided by our pharmacy benefit manager. When using the card at an Rx Consumer Card contracted pharmacy to obtain covered prescription medications, members will regularly save up to 22 percent on retail prescriptions. Outpatient prescription drug costs do not accumulate to the deductible or out-of-pocket maximum under this plan. |
| Behavioral health care and alcohol/chemical dependency rehabilitation abuse treatment.|
| Chiropractic and acupuncture treatment.|
| Maternity and newborn care.|
