Oral health affects both physical and mental wellness—and therefore quality of life. That’s why Anthem Blue Cross and Blue Shield offers the Anthem Blue Dental PPO plan for individuals and families.  
We designed our plan with two goals in mind: (1) To promote good dental hygiene and preventive care, important elements in a total health care package; and (2) To provide you with the dental benefits you need in a convenient, cost-conscious manner.  
 
With our plan, you have:  
The freedom to choose any dentist
Access to quality care at reduced fees
Coverage that includes a wide range of Dental services such as routine check-ups, cleanings, fillings, crowns, oral surgery, periodontal treatment, bridges and dentures
Both in-network and out-of-network benefits. That means you can see any dentist you choose, however, you’ll save even more when you choose a dentist from our dental network.
One of the largest Dental PPO networks in Colorado with more than 1,000 local dental provider locations and more than 75,000 locations nationwide, including general practitioners and specialists.
Preventive and diagnostic coverage that begins on your policy effective date
