The CoverMe Foundation Referral 
The CoverMe Foundation (CMF), a national nonprofit organization, has announced it is offering free, step-by-step assistance to uninsured consumers in identifying and applying for health care coverage as part of its mission to improve the overall health of Americans throughout the United States.  
The growing number of uninsured individuals is a major issue in our communities, but many of these individuals, or their family members, may be qualified to receive assistance. The foundation established by Anthem Blue Cross and Blue Shield’s parent company has funded The CoverMe Foundation, a non-profit organization committed to assisting uninsured individuals obtain healthcare coverage when eligible for Federal and State programs.   
In addition to the health care coverage assistance, CMF offers comprehensive health care information for often underserved groups, including the uninsured and underinsured, elderly and disabled, and children and pregnant women. It also provides information on prescription assistance programs and medical treatment programs.  
We invite you to use the referral fax/email form in the link below and fax or email, or provide the toll-free number, 1-877-NSURME1 (English) or 1-877-CUBREME (Spanish) to your patients who are uninsured, or have uninsured family members.   
Finding solutions to the issues of uninsured individuals is increasingly important as their numbers grow. It is estimated that as many as one-third of your patients without insurance are eligible for some type of assistance. Those who are pregnant, or live in a household with children under the age of 19, are even more likely to qualify. Specific government programs have been developed to assist working families.  
Be assured that the information that you fax/email will be treated with the utmost confidentiality by The CoverMe Foundation team, and the patients you refer will receive caring and sensitive assistance from professionals in eligibility enrollment. This information will be used for no other purpose than to screen the individual for available assistance through government programs.
