Lumenos® Consumer-Driven Health Plans

When it comes to health care, consumers often have little control or choice. Our Lumenos plans are changing that. With these consumer-driven health plans, consumers now have more control of their health, health care decisions and dollars.

Blue AccessSM (PPO)

Most network choice. This is Anthem’s broadest network of providers.

Blue Preferred® / Blue Preferred® Primary (HMO)

Most cost control. Employees must select a primary care physician (PCP).

Blue Preferred® Plus POS

Offers employers potential savings while offering employees more flexibility to choose doctors and hospitals.

Blue Traditional®

Unrestricted access. Employees may visit any doctor or hospital. In most situations, they pay a percentage copayment and deductible.

BlueWorldwide ExpatSM

BlueWorldWide Expat is a comprehensive system of worldwide, fully insured medical coverage for active workers in U.S. companies doing business aborad.

Anthem EssentialSM

Covers the most costly types of care emergency room, inpatient hospital and surgical care, including surgery in a physician's office.

Precertificatoin helps avoid unnecessary charges or penalties by ensureing that the member’s care is administered at a network facility and by a network provider. (Precertification guidelines) 
Anthem members have access to benefits when living or traveling outside Anthem’s Plan area* with the BlueCard® Program. 
 

*Anthem’s Midwest Plan area includes Indiana, Kentucky and Ohio.
