SmartSense®
For smart coverage at sensible savings, these plans come in a wide range of deductible/monthly premium combinations designed to give you lots of affordable options. Whichever plan you choose, you’ll have coverage for your first three network doctor visits including routine office visits – without having to meet your deductible first.
Premier
These are great family plans packed with benefits including low copays for doctor visits, plus comprehensive prescription drug coverage. Premier offers a wide range of deductible/monthly premium combinations. And routine preventive benefits you receive from network providers are not subject to your plan’s deductible. Optional maternity benefits are available on select plans, too.

Blue Short TermSM
Waiting for a new health plan to start? If you are temporarily without health coverage, a Short-Term plan can protect you from unexpected health care expenses for as little as one month up to six months. Designed to meet your needs while you are between health coverage options, these plans are not renewable.

Dental Blue® is an affordably priced dental plan designed to provide maximum choice, while offering you access to a full range of quality dental benefits.
Blue Preferred® Term Life
Blue Preferred® Term Life coverage can provide security and peace of mind for as little as a few cents a day. Three levels of coverage are available. Applying for Blue Preferred Term Life Insurance from Anthem is easy. Complete the Term Life Insurance section of your Individual Enrollment Application. If you are accepted for coverage, your life premium will be automatically added to your health insurance billing statement.

State Sponsored Plans- Indiana Hoosier Healthwise (Medicaid and SCHIP)
Indiana Hoosier Healthwise Program (Medicaid and SCHIP) gives health care services to those who qualify for state-funded programs in Indiana. State Sponsored Business (SSB) is a managed care plan that covers the health care benefits for Hoosier Healthwise members. SSB is known as Anthem Blue Cross and Blue Shield in Indiana.
