Know Your Health
The Anthem Blue Cross Companies' HealthyCheck Program offers members on qualifying contracts the option of receiving an annual preventive care screening that evaluates a variety of health risks. Screening results may confirm good health, identify where health may be improved, and may also reveal conditions that need further evaluation by your doctor.

$25 Basic Screening (for children ages 7-17 and adults 18 and over) includes

Other Things You Should Know
 | Trained, licensed health care professionals administer all screenings. |
 | They will review screening results and lifestyle recommendations with you at your HealthyCheck screening appointment. |
 | The flu shot offered is based on current recommendations from the Centers for Disease Control & Prevention (CDC) guidelines and provided by HealthyCheck centers that have an available supply. |
 | If you do not receive a flu shot during your screening because it is not flu season, you will be eligible for a future flu shot during flu season at no charge through a HealthyCheck Center. |
 | About 10 days before your appointment, you’ll receive a confirmation packet with instructions on how to complete your health status questionnaire (either paper or Internet version). |
 | As a Anthem Blue Cross PPO or EPO member, you are eligible for a HealthyCheck screening once every 12 months. |
 | The HealthyCheck screening is not intended to replace a full physical exam by your doctor. |
 | Coverage is provided or administered by Anthem Blue Cross and/or Anthem Blue Cross Life and Health Insurance Company - Please check your identification card |
Waiver of Deductible
Although most Preferred Provider Organization (PPO) policies provide coverage for annual Well-Woman and Well-Man exams and are not subject to the contract deductible, there are a few Small Group contracts, which fall outside of this provision. HealthyCheck Operations offers to waive the deductible for eligible Small Group members with qualifying contracts if they attend a HealthyCheck screening. HealthyCheck Operations will waive the deductible of both the office visit and lab for members with qualified Small Group contracts* for: Pap smears, mammograms, and PSA (Prostate Specific Antigen) tests.
* Some Small Group Contracts Provide waiver of deductible for lab work only. Please check your policy regarding coverage.
$25 Fee Waiver
As an incentive for members to participate in a HealthyCheck screening, a $25 fee waiver is offered to select ISG members. This offer is only good during the first year of enrollment and is only for one screening per newly enrolled Anthem Blue Cross or Anthem Blue Cross Life & Health Insurance Company PPO and EPO Plan member. A one time $25 credit is applied at the time the appointment is made. Offer good only at HealthyCheck centers, which may not be available in all areas served by Anthem Blue Cross and Anthem Blue Cross Life & Health Insurance Company. Offer is non-transferable.

Cancellation policy
Please call 1-800-274-9355 within 3 business days of your appointment if you are unable to attend. Members are only allowed to reschedule up to 2 times within a 12 month period.
