With one of our PPO (preferred provider organization) health care plans, you’ll pay a lower share of your medical expenses when you use doctors or hospitals that participate in our PPO network. Click on the links below for plan details.
ClearProtection
This plan offers a valuable combination of affordable coverage with some immediate benefits, plus a broad range of benefits once the out-of-pocket maximum is met.
CoreGuard
If you're looking for affordable protection against significant medical expenses, CoreGuard could be the plan that's right for you. CoreGuard offers a vast range of deductibles and higher cost-sharing to help lower your monthly premiums.
Premier
These are great family plans packed with benefits, including low copays for doctor visits, and comprehensive prescription drug coverage. Premier offers a wide range of deductible/monthly premium combinations. And routine preventive benefits you receive from network providers are not subject to your plan’s deductible after you've been on the plan for six months.
Individual Lumenos® HSA, HIA and HIA Plus health plans
Lumenos plans focus on keeping you healthy while giving you more control over your health care decisions and dollars.
PPO Share Plans
These are our most comprehensive PPO plans with rich benefits and a choice of three deductibles. Ideal for families with young children, these plans feature immediate benefits (deductible waived) for doctor’s office visits, an annual physical exam and preventive care. Prescription drug and maternity benefits are also included.
SmartSense®
One of our lowest priced plans that offers solid protection to cover the essentials and offers a choice of four deductibles. Your first three doctor office visits are covered before your deductible and you have a choice of two prescription drug coverage options. If you don’t need maternity coverage, this plan could be ideal for your needs.
RightPlan PPO 40
With no medical deductible, this simple plan features a $40 copay for in-network doctor visits. And this plan offers you a choice of three prescription drug coverage options.
