About our Preferred Drug Lists or “Formularies”
Our health plans use a "Preferred Drug List" containing brand-name and generic medicines approved by the Food and Drug Administration (FDA) and reviewed and recommended through our Pharmacy and Therapeutics (P&T) process. Our P&T process is performed by an independent group of practicing doctors, pharmacists and other health care professionals responsible for the research and decisions surrounding our drug lists. This group regularly meets to review new and existing drugs and chooses the medicines for our drug lists for several reasons, including their safety, value and how well they work.

Select Drug Lists
If you purchased your prescription drug coverage on the health care reform exchanges, you should use our Select Drug List to determine your coverage. Reviews and recommendations for the Select Drug List are also done through the P&T process described above.
What if I am prescribed a medication that is not on the Select Drug List?
You or your health care provider may request an exception based on certain criteria. Please review the criteria below and use the Online Formulary Addition Request Form to request drugs not found on the Select Drug List.
