Behavioral Health benefit plans are an integrated part of the Anthem Blue Cross medical product line. Our members have access to over 8,000 psychiatrists, psychologists, and therapists statewide. Our Behavioral Health Network is made up of a multidisciplinary panel of behavioral health specialists who provide mental health and/or substance abuse treatment in accordance with member benefit plans. We are committed to providing quality behavioral health care for our members through our diverse Behavioral Health Network.

A request for consideration is a part of the initial review process to determine whether an application will be offered to you. It is important to understand that the initial review process is a separate process and that not all providers will receive an application for participation.
If an application packet is not received, a network and/or specialty need has not been identified at this time.

Current Resume or curriculum vitae (CV)
Individual malpractice insurance facesheet
Copy of State license
| For licensed practitioners that provide Applied Behavioral Analysis (ABA) services, or who supervise Board Certified Behavior Analysts (BCBAs) or practitioners with similar training that provide ABA services, also include a cover letter specific to:|

After I submit my request for consideration, what happens next?
| Once you submit your request for consideration, the information will be reviewed carefully. In order to meet the needs of our members, it is important that our behavioral health practitioners meet educational, training, certification/licensure and/or other requirements. |
| If a network and/or specialty need is identified, we will forward a contract and an application packet for you to complete and return promptly.|
| Complete the credentialing application process following the instructions provided. CAQH website is CAQH.org , click ‘Providers’ tab.|
| Note: You must successfully complete the application/credentialing process in order to become a participating practitioner with Anthem Blue Cross.|
