Dental Blue® PPO Plans
We offer two Dental Blue PPOs for individuals and families – Dental Blue Basic and Dental Blue Enhanced.
Dental Blue Basic offers coverage for most diagnostic and preventive services such as cleanings, exams and X-rays, as well as coverage for fillings, stainless steel crowns and pulpotomies for baby teeth. There is a six-month waiting period for fillings. Out-of-network services will be paid based on the Dental Blue 100 fee schedule. This plan has an annual maximum of $500.
Dental Blue Enhanced offers coverage for most diagnostic and preventive services, basic services and major services like crowns, periodontal procedures and root canals. There is a six-month waiting period for basic services (such as fillings) and a 12-month waiting period for major services. The plan has an annual maximum of $1,250, plus it includes orthodontic coverage for children after a 12-month waiting period.
A key feature of our Dental Blue plans is that members can take advantage of the rate we’ve negotiated with dentists for:
Services received during waiting periods
Services received after they’ve reached their annual maximum
Non-covered services, including cosmetic services like veneers
To further simplify things, both of our Dental Blue PPO plans will use the Dental Blue 100 network. Over the last year, our Dental Blue 100 network has grown 45 percent
to more than 18,300 participating providers. And members have access to more than 49,000 Dental Blue 100 providers
Dental DHMO Plan
The Dental SelectHMO dental plan offers great coverage with lower out-of-pocket costs and features:
No annual maximums
No annual deductibles
No charge for cleanings, exams or X-rays*
Most fillings are covered with no copayment after a six-month waiting period
Orthodontics and other services are available for fixed copayments
International Emergency Dental Program
Beginning March 1, 2010, all Anthem Blue Cross and Anthem Blue Cross Life and Health Insurance Company dental members will have access to emergency dental care while traveling or working abroad through the International Emergency Dental Program administered by DeCare Dental.** This new benefit is automatically included at no additional cost for all individuals with dental coverage.
Why? We know it’s important for our members to have access to emergency dental treatment when they’re far from home, and this new benefit gives them access to a worldwide listing of credentialed dentists.
Out of Network Reimbursement
It is important to understand how an in-network dentist may be able to help you save money. In-network dentists have agreed in a contract with us to accept specific amounts for various dental services. Because there is no similar contract with out-of-network dentists, they can bill you for the difference between what we pay and what they charge.
