When you enroll in our Lumenos HIA Plus plan, you’ll get:
Health Incentive Account (HIA) Plus
An account funded by the health plan each year to use on covered medical expenses and prescriptions.  
Use your HIA funds first to pay for covered medical expenses and prescriptions.
Unused funds rollover year-to-year for most plans.
Unlimited choice of providers. If you see in-network providers, you may pay less for your care.
Plus, get rewards* – additional funds added to your account:

$50 for completing the MyHealth Assessment

$100 for enrolling in the health coach program

$200 for graduating from the health coaching program

$50 for completing the Healthy Lifestyles: Tobacco-Free program

$50 for completing the Healthy Lifestyles: Healthy Weight program

Plus, get 100% coverage for nationally recommended preventive care services with no deduction from your HIA or out-of-pocket costs for you if you see an in-network provider.  
Traditional Health Coverage
A comprehensive health plan that provides added protection for you and your family, after you use all of your HIA funds.  
Traditional Health Coverage begins after you have used your HIA allocation and paid an out-of-pocket amount – called your Bridge – needed to satisfy your plan’s annual deductible.
Your out-of-pocket expenses are limited to a maximum each year, after which you receive 100% protection for covered services.
