CU Health Plan - Exclusive is a limited network plan that extends throughout Colorado. All members must select a primary care physician (PCP) and your PCP will direct your health care as appropriate. Please note that while referrals are needed for specialists' visits, you do not need a referral to see an in-network obstetrician-gynecologist (OB/GYN), chiropractor, acupuncture provider or an in-network ophthalmologist. PCP changes can be made at any time and will take effect the 1st of the month following the date of change.

CU Health Plan - High Deductible/HSA Compatible is an HSA qualified consumer directed health plan that offers a national network of providers, and is available in Colorado, the United States and worldwide. Plan members can refer themselves to doctors of their choice, including specialists, both in-network and out-of-network; however, higher out-of-pocket costs are incurred when using out-of-network providers. Preventive care is covered at 100% in most cases.

CU Health Plan - Extended is a PPO plan that offers a National Network of Providers, and is available In Colorado and throughout the United States. Plan members can refer themselves to doctors of their choice including specialists. There are no out of network benefits except for Urgent and Emergent Care, Durable Medical Equipment, Lab and X-ray. Preventive Care is covered at 100%.

CU Health Plan - Medicare provides benefits to University of Colorado Retirees and Surviving Spouses who are Medicare eligible and enrolled in Medicare Parts A and B. Members can utilize any provider that accepts Medicare assignment in Colorado and throughout the United States. Please Note: Only available to eligible 401(a) CU and UPI retirees. Not available to PERA CU Retirees.

CU Health Plan - Vision provides two vision options for CU Health Plan members: a $20 Copay Exam Only plan and a Voluntary Exam and Hardware plan. The $20 Copay Exam Only plan is only available through the CUHealth Plan - Exclusive/Exclusive2 plan. The voluntary Exam and Hardware plan is available to all members, except for UCHealth Employees, regardless of medical enrollment.
The vision plans are made available through Anthem’s Blue View Vision products and utilize the EyeMed national network. The provider network consists of independent optometrists, ophthalmologists and opticians as well as convenient national optical retailer stores many of which offer convenient weekend and evening hours.
