Anthem Blue Cross and Blue Shield Foundation Funds Diabetes Program
Wise, VA.--April 18, 2013 – The Health Wagon is pleased to announce that the Anthem Blue Cross and Blue Shield Foundation has provided a $200,000 grant to support the “Decreasing the Prevalence of Diabetes in Southwest Virginia” program. This grant will support the Health Wagon’s initiative to help manage diabetes, decrease risk factors, and support screening initiatives to identify and treat new diabetics.
“The Health Wagon is honored to receive support from the Anthem Blue Cross and Blue Shield Foundation to support our diabetes program. This donation promotes and enhances access to health care delivery for citizens of Southwest Virginia,” said Dr. Teresa Gardner, executive director of The Health Wagon. “The donation supports the Health Wagon to continue valuable and needed services.”
Diabetes has become an epidemic in Southwest Virginia, resulting in increased morbidity and mortality rates for its residents. With the assistance the Anthem Blue Cross and Blue Shield Foundation, the Health Wagon hopes to change the course of this debilitating disease. The funding will allow the Health Wagon to screen over 3,000 patients for diabetes, provide care for their current diabetes population, provide educational services for diabetics on therapeutic lifestyle changes, identify patients with diabetes and provide chronic care management utilizing evidenced based chronic care models.
“We are pleased to help the residents in Southwest Virginia confront diabetes by partnering with the Health Wagon on this program,” said Jay Schukman, M.D., chief medical director, Anthem Blue Cross and Blue Shield. “Chronic conditions like diabetes can be debilitating and devastating. However, by focusing on early detection through screenings and treatment, we can help increase the likelihood diabetes can be managed.”
