An employee may waive (refuse) the coverage for which they are eligible. To waive coverage, the employee must complete portions of the Employee Application. The sections of the application that must be completed are:
Social Security Number
Waiver of Coverage Box
Signature and Date
Completing these sections of the application acknowledges that the employee was given an opportunity to enroll and chose not to enroll.
The employer should send the completed application waiving coverage to Anthem Life and keep a copy for their records, the same as any other employee application.
