Certificates, amendments and coverage notices are available to all Anthem group and non-group subscribers and to all HMO Colorado group subscribers at anthem.com. Subscribers may also request a printed copy of their certificate by mailing the postage-paid postcard included with their health plan ID card(s), or by calling the customer service number on their ID card. The certificate, amendments and coverage notices explain the type of coverage and benefits available to the member, as well as limitations and exclusions.
Anthem mails health plan ID cards to all Anthem and HMO Colorado groups (i.e., to the employer or to the group subscribers) and to all non-group subscribers. Some local Colorado member health plan ID cards list an issue date and an effective date. The card issue date is the date the card was printed. The effective date is the date the benefits under the member’s certificate were available to the member. With each visit, please ask members for the most current copy of their health plan ID card.
Member health plan ID cards include information about verifying member eligibility. Possession of a health plan ID card does not guarantee that the person is an eligible member. If a member does not have a health plan ID card, please contact Anthem customer service or BlueCard®
eligibility at the phone numbers in the Telephone/Address Directory
Anthem’s verification of eligibility is effective as of the date the eligibility is verified. Eligibility may change between the time it is verified and the date services are rendered. If a Provider verifies eligibility within two business days before rendering a service, payment for that service will not be denied if the member’s eligibility later changes. If Anthem denies a member’s eligibility, the member is responsible for payment if services are provided.
The design of BlueCard member health plan ID cards will vary from Blue Cross and/or Blue Shield Plan to Blue Cross and/or Blue Shield Plan, please see the BlueCard section
for sample ID cards.
