Help our members prepare to renew their Medicaid healthcare coverage and benefits by educating them on what documents are needed to complete their renewal form accurately. Members can submit their renewal form via the web (preferred), phone, fax, mail, or in person.

Indiana PathWays for Aging
Indiana PathWays for Aging is a statewide coordinated care program for Indiana’s Medicaid enrollees who are 60 years of age or older, and eligible for Medicaid based on age, blindness, or disability, and have limited income and resources. Anthem supports members enrolled in Indiana PathWays for Aging by implementing strategies to address complex and chronic health conditions.
