Patient360 is a read-only dashboard that simplifies administration by coordinating access to member-centric patient information, allowing providers who are part of a patient’s care and service team to easily access the patient’s record through a self-service website.

Log in to Availity 

Access Patient360 on Availity Essentials

To use the Patient360 tool:

1. From the Availity homepage, select Payer Spaces from the top navigation.
2. Select the health plan.
3. From the Payer Spaces homepage, select the Applications tab.
4. Select the Patient360 tile.
