Starting January 1, 2025, we will enhance your healthcare experience by transitioning your pharmacy benefits to CarelonRx Pharmacy. We are expanding our partnership with Anthem to better serve you.

If your prescription will be impacted by the change, DaVita will be offering time throughout 2025 for you to work with your doctor to determine the best covered alternative for you.
