Earn up to $50 in Healthy Rewards
Complete the Health Needs Screening (HNS) within 30 days (for PathWays members) or within 90 days (for Hoosier Healthwise, Healthy Indiana Plan, or Hoosier Care Connect members), and we’ll give you $50 in Healthy Rewards. See the Healthy Rewards page for more information on program eligibility.
