Care Coordination

You’ll have your own Care Coordinator who will get to know you, your family, and your care team. Your Care Coordinator will talk with you over the phone or visit with you in person to help you:

• Learn more about your health condition(s)
• Get answers to your questions about medicines
• Get the medical supplies and equipment you need
• Find doctors and other resources close to home
• Coordinate all of your physical health, behavioral health, and social needs

Your Care Coordinator will help you to create a person-centered care plan designed just for you. You and your Care Coordinator will help to make sure you understand all of the resources available and work together to figure out the right combination of services and supports you need to help you meet your healthcare goals.

If you are enrolled in the home- and community-based services waiver, you will have a Waiver Service Coordinator. Your Waiver Service Coordinator will help you coordinate all of your waiver services through a waiver service plan. Your Waiver Service Coordinator will:

• Assess your needs, goals, and objectives at least annually
• Coordinate with you, your Care Coordinator, and your care team
• Work with you and your care team to develop your waiver person-centered service plan
• Authorize waiver services in the amount, scope, and duration to meet your needs
• Monitor the delivery of all waiver services identified in your waiver service plan

If you are enrolled in the home and community based waiver, you can choose to self-direct some of your waiver services. Your Care Coordinator and Waiver Service Coordinator will provide education regarding this option and support you through the enrollment process.
