Renew Your MyCare Ohio Benefits
Keep your coverage. Be sure to renew your MyCare Ohio benefits every 12 months.

Ready to renew?
It’s quick and easy. Follow these steps to renew your benefits:
1. Check your mailbox
You’ll get a renewal letter from the County Department of Job and Family Services telling you it’s time to renew.
2. Fill out your renewal form
Complete the entire renewal form and gather any documents requested. You may also complete your renewal form online.
3. Send it in
- Submit your renewal online on benefits.ohio.gov.
- Call the Ohio Medicaid Consumer Hotline at 800-324-8680 Monday through Friday from 7 a.m. to 8 p.m. or Saturday from 8 a.m. to 5 p.m. Eastern time.
- Bring it in person to your local eligibility office.
Keep your address up to date
If you moved recently, call your local county Job and Family Services office to give them your new address. Remember, you also need to update your address with the Centers for Medicare & Medicaid Services (CMS).

What if you lose your coverage?
If you miss your renewal deadline, you have a limited amount of time to reapply after the date your coverage ends. Visit benefits.ohio.gov to reapply today.
Keep in mind, if you are a D-SNP member who has both their Medicare and Medicaid benefits through Anthem MyCare, you may lose your Medicare coverage if you do not reenroll in Medicaid soon.
