Condition Care is all about you
Our Condition Care (CNDC) programs allow you to ask questions about your condition, get benefit information, and connect with local resources.

Our CNDC Care Coordinators are registered nurses. When you join one of our CNDC programs, a Care Coordinator will:
Who can join?
Anthem MyCare members with any of the health conditions listed above can participate in a CNDC program. A CNDC Care Coordinator will talk with you about your health history and health goals. Then they’ll work with you to find solutions so you can live well.
Our Care Coordinators also offer tools for weight management and tobacco cessation (how to stop using tobacco, such as quitting smoking).

You may also call this number to leave a private message for your CNDC Care Coordinator 24 hours a day.
Your rights and responsibilities
When you join a CNDC program, you have certain rights and responsibilities. You have the right to get details about us, such as:
- Get help on how to use the complaint process.
- Know how much time Anthem MyCare has to respond to and resolve issues of quality and complaints.
- Give us feedback about the program.
You also have a responsibility to:
