On an HMO plan, you’ll choose a primary care physician (PCP) who you’ll see for all your check-ups and regular exams. Your PCP also directs you to the right specialists when you need them.

On an Anthem PPO, you’ll have lots of flexibility. You’ll still pick a primary care physician for check-ups, but you can also choose to see any doctor or specialist in your network without needing a referral. At Anthem, our network is always growing to serve you better. You can also see doctors outside your network but services may cost more.

Special Needs Plans

Anthem’s Dual Eligible Special Needs Plans (DSNPs) in Indiana are designed for people with both Medicare and Medicaid. Special Needs Plans are there to help you manage your health or health conditions. They’re HMO plans with prescription drug coverage that’s included. Plus you get lots of extra benefits to help keep you healthy, like car rides to doctor visits, an allowance for eyewear, hearing aids, and health items, and $0 copays for extra vision and dental coverage. You’ll even have access to our popular fitness program, SilverSneakers®. These Anthem plans also have premiums for $0 with no medical copays or deductibles.
