Medicare Advantage Dual Special Needs Plans are for people eligible for both Medicare and Medicaid. These plans offer coordinated care and additional coverage not available with Original Medicare. An Indiana D-SNP from Anthem can make life easier and help you stretch your budget.

Indiana PathWays For Aging And Fully Integrated Dual Eligible (FIDE) Plans

 

If you are enrolled in or eligible for both Medicare and the Indiana PathWays for Aging (PathWays) program, Anthem’s PathWays for Aging Dual Care (PathWays Dual Care) plan might be a good choice for you.

 

PathWays is a program for Hoosiers aged 60 and over who receive Medicaid (or Medicaid and Medicare) benefits offered by the Indiana Family and Social Services Administration.

 

With an Anthem PathWays plan, we’ll help you get the services and support you need to live as independently as possible. The program will not change or expand your Medicaid benefits. Each qualifying Hoosier will have a care and services coordinator. These coordinators will help you get all the benefits you qualify for.

 

The Anthem PathWays Dual Care plan is a Fully Integrated Dual Eligible (FIDE) plan, which means all your care for both Medicaid and Medicare benefits are seamlessly managed by Anthem.

 

The Anthem PathWays Dual Care Advantage NFLOC (HMO D-SNP) is for people that need a Nursing Facility Level of Care.

When Can I Enroll In A D-SNP?

If you have Medicare Part A and Part B, you can enroll in a Dual Special Needs Plan the month you become eligible for Medicaid.

 

If you are already enrolled in a D-SNP plan, you may be able to change plans monthly depending on your current plan and/or the plan you want to enroll in. And if you've had a life event such as moving, losing coverage or newly eligible for Medicare, you may qualify for a Special Enrollment Period. You can always change plans during the Annual Enrollment Period from October 15 to December 7 every year.
